All information you submit becomes the property of The ON/AVN SGIA, Inc., and will be used at some point for research purposes. You do not have to give your own personal identifying information; all surveys will remain anonymous. However, as in the case of our Doctor Database Survey, we do ask that you give your city and state, or country and province, to help direct others in their search for doctors in their areas.
